Before committing to the practice, we invite you to come meet a provider, see the office, and ask any questions. There’s no charge and no obligation, just an easy way to find out if Mountain Ridge is the right fit for your family.
If a video visit is easier, we can do a “Meet the Doc” by Doxy.me instead. Either way, please call our front desk to schedule.
Complete a few forms.
Filling these out before your first visit means more time with your provider and less time at the front desk.
If you’re moving to a new practice, you can also complete the “Transfer Records Out” form from our Patient Forms page. Let our front desk know if you need help with any of these.
What to bring to your first visit.
A short checklist so check-in is quick and we can spend the most time with your child.
- Completed forms. The registration, transfer-in (if applicable), and HIPAA acknowledgment from Step 2.
- Insurance card. We’ll make a copy at check-in.
- Photo ID for the parent or guardian.
- Immunization records. If your previous practice didn’t already send them, please bring a printed copy.
- A list of medications & allergies. Including dose and frequency for any current prescriptions.
- Your questions. Write them down ahead so nothing slips your mind.

Same-day sick visits
Call first — we’ll work to fit your child in the same day for fevers, coughs, ear pain, and other acute issues.
Telemedicine when it’s a fit
Connect with your provider by video from anywhere. Telemedicine is billed through insurance per your plan’s coverage.
After-hours support
Reach our on-call provider by call or text at (540) 249-1110 when something can’t wait. ($20 fee, effective June 1, 2025.)
